Incident Reporting

Performance concerns should be brought to the attention of the Immersion Faculty by the Project Mentor. An informal conference with the student will be scheduled to discuss the concerns and provide constructive feedback. 

Should the performance concerns persist after this informal conference, the immersion faculty will formulate a written plan explaining the areas of concern and the behaviors necessary to correct these deficiencies. This process will be completed prior to the end of the course, allowing sufficient time for the student to improve. The immersion faculty will contact studentservices@usuniversity.edu after communicating with the Project Mentor to acknowledge the identified concerns and develop a written remediation plan. 

• If the conditions of the remediation plan are not met by the student by the last immersion day, the student’s immersion performance will be unsatisfactory on the final Practice Immersion evaluation, and the student will receive a failing grade. At any time, if a student’s Immersion performance in an Immersion course indicates an inability to perform at a safe and/or professional level of practice, the Practice Immersion faculty will assign a failing grade regardless of the point in time such a decision is made. In such case, the student will be ineligible to continue in the course.
