Policy: All forms used to interact with the IRB are maintained via links within this policy. Each form includes a link dedicated to that form for submission to the IRB. Forms used internally (for example, IRB Reviewer Checklists) are maintained on the IRB Google Drive.
Purpose: The purpose of the policy is to house current IRB Forms used by Investigators.
Procedures:
- Investigators complete the relevant form accessible within policy 700-41 and submit using the link on the form.
- Each form is processed per the policy related to that form.
- IRB Members and Staff access relevant form via the IRB Google Drive
Scope: This policy applies to all forms used by investigators conducting research or projects at USU.
