Health and Safety Requirement Descriptions

Health and Safety documents must be current and compliant for continuous enrollment and/or to avoid practice immersion restrictions or delays.

Health and Safety documents are submitted into Complio. Complio organizes, tracks, communicates, and shares practice immersion requirements and information through their platform. Complio has review criteria that must be met, such as clear/legible documentation.

Students are responsible for maintaining compliance in advance and throughout their practice immersions. Complio communicates upcoming expirations at 60, 45, 30, 15 days in advance and day of. Once the item has expired, OFE is notified and will move the student into not compliant status. A notice goes out to the student that they shall not go to practice immersion/log any hours.

It is imperative that students update their documents in advance to avoid delays. Students who choose not to proactively update their requirements must allow Complio and OFE to carry out the appropriate processes to ensure safety and compliance for students. Urgent/Rush requests cannot be accommodated due to the nature of these reviews. Students are responsible for any financial implications resulting from late or lack of submission of documents. Processing times can vary based on the requirement but below is the general timeline.

Upcoming Expiration notices begin 60 days prior to an item expiring from Complio.

  • Item Expired and student receives notification of expired item from Complio, OFE and Project Concert. The student is marked as non-compliant in Project Concert. Faculty have the ability to see the students compliance status and compliance history in Project Concert. The students are sent a notice to not attend practice immersion or log any hours while non-compliant. They are advised to reach out to their Clinical Placement Coordinator for questions.
  • Once the student submits the item, Complio generally needs 2-5 business days to review.*
  • If Complio denies the submission, the student will need to resubmit an item that meets the requirements. If Complio issues an approval, OFE is notified.
  • OFE reviews and updates students compliance status under the Manual Process tab within 2 business days.**
  • Students receive notice from Project Concert that they are compliant and can start or resume practice immersion.

STUDENTS ARE NOT COMPLIANT UNTIL THE ENTIRE PROCESS IS COMPLETED.

*Depending on the requirement, this process can take significantly longer. For example, some states have strenuous background check requirements which can take months to process.

Additionally, OFE works proactively to identify upcoming expirations for agreement and project mentor documents. However, from time to time they can lapse. OFE will communicate with the student in those situations as we are dependent on response from the site/project mentor for resolution and cannot predict timeline.

Students should ensure their email used in Complio is up-to-date and actively monitored. Students are responsible for Complio fees. Sites may require additional health and safety requirements that must be met.

Description

Tuberculosis (TB)

For students who test Negative for TB exposure, you must submit A or B:

A) Negative 2-Step PPD, where the 2nd Step is dated with 7-365 days of the 1st Step. Annually thereafter, only an Annual 1-Step PPD is required.

B) Negative Quantiferon TB Gold Test / T-Spot each year.

If you have a history of a Positive TB test, you must submit all of the below:

Proof of that Positive TB Test

Negative Chest X-Ray, which will expire every 5 years

TB Health Evaluation, due each year.

Note: If a student chooses to submit QuantiFERON®- TB and then it expires and the student chooses to then submit Negative 2-step PPD, both steps are required (i.e. QuantiFERON®- TB will not be acceptable as the first step)

BLS Card

American Heart Association BLS certification for the Professional Rescuer or Healthcare Provider is required. This certification is valid for a two-year period.

Government/military issued BLS may be acceptable. All other BLS courses will NOT fulfill this requirement. Online-only courses are not acceptable. ACLS alone is not acceptable.

Tetanus-Diphtheria (Tdap)

Documentation of Tetanus-Diphtheria with Acellular Pertussis vaccination administered within the past 10 years. (Tdap vaccines required, TD can be provided after Tdap expires.)

 Influenza (Flu)

Flu vaccines are available from late September through early May. Students are required to have a current flu vaccine on file in order to enter the clinical setting. Influenza vaccine is required annually during flu season.

Nursing & APRN License & Board Certification

Students are required to carry an active, unencumbered RN and APRN nursing license from the state that student will anticipate performing their practice immersion rotation in. Student’s nursing licenses and liability insurance coverage should both be active and within the same state. Any changes to a student's licensure status must be reported to the Program Director immediately.

Students should also provide proof of US national board certification.

Medical Clearance Form

A physician or other health care provider completes this form following a physical exam of the student.

Hepatitis B

Students are required to submit the Hepatitis B 3-shot series or a Positive Hepatitis B (HBsAb) Titer. If the titer is Negative or Equivocal, you must submit the 3-shot series (4th, 5th, & 6th) dated AFTER non-immune titer date. Heplisav-B does not meet the requirement.

MMR: Measles, (Rubeola), Mumps, Rubella

Students are required to submit either 2 doses of MMR OR Positive Titers for Measles, Mumps, and Rubella. If a titer for any disease is Negative or Equivocal, you must submit 2 doses of MMR dated AFTER the non-immune titer date. It is not sufficient to report having had any of these illnesses or vaccinations as a child.

Varicella (Chicken Pox)

Students are required to submit either 2 doses of Varicella OR a Positive Varicella titer. If the titer is Negative or Equivocal, you must submit 2 doses of Varicella dated AFTER the non-immune titer date. Documentation of history of chicken pox or varicella vaccinations are not sufficient

 Health Insurance

Verification of the student’s current and active health insurance coverage. (e.g., copy of insurance card or online verification screenshot.) Military/government ID does not fulfill this requirement.

Malpractice/University Insurance & Student Self Coverage

OFE does not require a specific provider. DNP students are required to carry their own professional liability insurance. The coverage is a minimum of $1,000,000 per event/$6,000,000 aggregate. This coverage must state that the student is in an advanced practice/nurse practitioner role and should be independent coverage, meaning it should not state the insuree is fulltime employment or list any business/organization as the insuree.
CONHS carries Medical Professional Liability Insurance on all students in programs that require practice immersion experiences. The coverage is $1,000,000 each occurrence and $3,000,000 aggregate.

Background Check

Students must complete and clear a background check prior to first clinical rotation. No felonies are accepted and misdemeanors will be subject to review by the Program Director whose decision is final. A student must report, within 10 days, any legal violation such as, but not limited to, a DUI, misdemeanor or felony if occurrence is during the program. Students will not be permitted to continue in the program without a disposition date in any circumstances. Students may not be allowed to continue in the program depending on the offense. 

Hepatitis A

Recommended but not required.

Drug Screen

Students are required to have a negative drug screen prior to beginning first clinical experience. Students will be notified by the Office of Field Experience or Complio when the drug screen is available or required. Students should refer to the detailed Policy for Student Drug, Nicotine & Alcohol Screening section of this Handbook for further details

HIPAA Form

Students must sign and return this electronic form, which certifies the student has completed the HIPAA training within the Nursing Caring Corner and completed the quiz with an 80% or higher.

Universal Precautions

 Based on the mandatory CDC and OSHA guidelines, students and clinical faculty will consistently observe blood and body fluid precautions when working with all clients in all settings. Students are required to complete the Blood-borne Pathogens Training in the Nursing Caring Corner. Once training is completed, students must take quiz and get an 80% or higher. Students will submit a screenshot of the grade page (showing name, date, and grade) to meet this requirement.

COVID-19 testing or vaccine

Students in a clinical course must meet one of the following as of October 15,2021:

A) 2 doses of Moderna or Pfizer COVID-19 vaccination.

B) Single dose of the Johnson and Johnson COVID-19 vaccination.

The Novavax Booster also meets this requirement.

If a student chooses to decline the vaccination, they must request an exception in Complio. They will need to include the USU Covid-19 declination form, sign it, and upload it to Complio.
