Policy: Student Photo Identification Cards (ID) serve as a way to verify a student's identity when attending on or off-campus activities. To obtain a student identification card, please complete the Student ID Request Form. Students will need to include Full Legal Name, mailing address and a passport style photo. Students may be required to pay a fee to obtain a photo ID as well as a replacement fee. Please refer to the Tuition and Fee section of this catalog.
Purpose: To serve as student identification for a variety of on-campus services, access to the building during business hours.
Procedure:
- To obtain a Student ID, students must submit a request using the online Student Identification Card Request Form.
- Students must include full legal name, mailing address and a passport style photo.
- Upon submission, the Office of the Registrar will process the request and mail the Student ID card within 7-10 business days to the address provided on the request form.
- Once printed and mailed, students will receive an email notification confirming the successful processing of their Student ID.
- In cases where students require a duplicate Student ID, a fee will be required. Please refer to the catalog for the current fee amount. Students can make the payment for the duplicate ID via their student portal.
Scope: The scope of this policy applies to all students.
