Policy: The University requires that all new students obtain a student ID card. The cards serve as student identification for a variety of on-campus services, access to the building during business hour, and the University Library. To obtain a student identification card, please contact Academic Advising: studentservices@usuniversity.edu and an academic advisor will provide link to the Student ID Request Form. Students will need to include Full Legal Name, mailing address and a passport style photo. Students who lose their ID may be required to obtain a replacement and pay the replacement fee. Refer to the Tuition and Fee section of this catalog.
Purpose: To serve as student identification for a variety of on-campus services, access to the building during business hours, and the University Library.
Scope: The scope of this policy applies to all University students.
