Policy: This policy outlines the procedures and guidelines surrounding the issuance of diplomas, certificates, transcripts, and other official documents. Written consent is required in accordance with FERPA regulations before any student information can be released. Please refer to the Tuition and Fees section for costs associated with transcript and other requests.

Diplomas/Certificates- A diploma/certificate is a formal document awarded to students upon successful completion of their academic program.

Official Transcripts - An official transcript is a record of a student's academic history, including courses taken, grades received, program enrollment, and degrees conferred.

Other Official Documents - Additional official documents include enrollment verifications, degree verifications, and grade reports.

Purpose: The purpose of this policy is to inform students on the process of requesting official documents from the university.

Procedure:

Diplomas/Certificates

  1. Approximately two (2) months after successful completion of a program, diplomas/certificates are mailed to the address provided on the Application for Graduation. Replacement Diplomas are available upon request through Diploma Services and are subject to a replacement fee.

Transcripts 

  1. Students may request official transcripts online through Transcript Services.
  2.  Official transcript requests will be processed within 48-72 business hours.

Other Official Documents

  1. Students that require documentation and/or unofficial transcripts may complete the Document Request Form and submit to registrar@usuniversity.edu.
  2.  All requests will be processed within 48-72 business hours.

Scope: This policy applies to all former, current and future students.
