Organization Permission Letter

Many projects/studies occur in collaboration with another organization. For example, a DNP student may conduct their project at a clinic, while an EdD or DBA student may conduct their study via a Facebook or LinkedIn group or at a specific workplace. As such, you will need to submit permission from the organization(s) with your IRB materials.

Note to DNP candidates: The Organization Permission Letter is not the same thing as a Practice Immersion Site Agreement.

In general, the Organization Permission Letter must be printed on letterhead* and include the following:

  1. Your name and affiliation with United States University
  2. The title of your project or study
  3. A 1-3 sentence purpose
  4. A statement that your protocol has been reviewed by the person granting permission
  5. A statement about what is being permitted at the site and if applicable, statements about any restrictions
  6. The title of the person granting permission
  7. The email address and phone number of the person granting permission
  8. A signature of the appropriate person who attests that they have the authority to grant such permission*
