IRB Submission Checklist

What you submit to the IRB reveals a great deal about you. Give the IRB confidence about your professionalism and capabilities based on a well-written, accurate submission. If your IRB Review Form is incomplete, contains errors/typos, or provides inconsistent information, your review will be delayed. Thus, ensure your submission is complete and free from inconsistencies, typos, or grammatical and spelling errors. Further, typos and errors in your recruitment materials, informed consent form, or instruments may steer potential participants away from your study or project or cause them to question your credibility. For this reason, and because your status as a doctoral student reflects on the reputation of USU, the IRB may ask you to revise materials that will be seen by potential participants for accuracy and clarity.

 

You will send all documents to your chair who will review and provide feedback. When all your documents are ready, they will submit it to the IRB on your behalf.

  • Zipped files are not accepted
  • Each submitted document should be named per the USU naming convention: First InitialLast Name_Weekly Assignment_Course Code (e.g., JFulginiti_WeekX_IRBForm_DBA7XX)

 

The following are required with every submission:

  • IRB Review Form (the Form required for your program is available in your Doctoral Community as well as within policy 700-41).
  • Three CITI Training Certificates of Completion: (1) Researchers - Information Privacy & Security (IPS), (2) Social and Behavioral Responsible Conduct of Research, and (3) Social-Behavioral-Educational (SBE) Comprehensive
    • CITI training is valid for three years. Because the letter from the IRB granting you permission to conduct your project/research is valid for one year, your CITI training must be in effect during this time. Check to confirm that your expiration dates are at least one year out from your submission date. If they are not, you will need to repeat the training. 
    • You will need the Certificate of Completion for each course (not the Completion Report)

If applicable, you will also submit:

Organization Permission Letter in the form of a signed letter on official letterhead from the organization giving approval to implement your activity in the target setting (Note this is required for all DNP students)

Informed Consent Form

☐ Approval letter from CIR if you are proposing to conduct research on USU constituents (see policy 211)

HIPAA Authorization Form (fillable PDF available in the Doctoral Community)

Participant Recruitment Materials (e.g., flyer, social media post, email)

IRB approval(s) from other IRB(s) (e.g., practice site; another college/university)

A copy of your instrumentation (e.g., survey questionnaire, interview questions, protocol, screening tools)

If instruments were purchased, a copy of the proof of purchase

If instruments require permission to use or modify, a signed permission letter or email from the appropriate person

Any other materials that will assist the IRB in the review, for example, a protocol, training, checklist, explanations related to the use of deception, vulnerable populations, etc.
