Policy: United States University expects its administrators, faculty, staff, and students to adhere to all university policies. Occasionally, exceptions to a policy may be granted based on criteria established in each policy and related procedures, or as determined by the Provost or President as described in this policy and related procedures.

Purpose:  The purpose of this policy is to describe how exceptions to a university policy are handled.

Procedure:

1. If the procedure for an exception is established within the policy in question, that procedure is followed. If no procedure is established, the Exception to Policy Form is used.

2. The USU constituent requesting the exception completes the form and emails to registrar@usuniversity.edu.

3. The Registrar reviews the form, completes the appropriate section, and forwards to either the Dean (academic policy) or the Policy Owner (all other policies).

4. The Dean/Policy Owner complete the appropriate section and forwards to the Provost (academic policy) or President (all other policies).

5. The Provost/President either approves or denies the request and returns the form to the Registrar.

6. The Registrar informs all parties of the decision and facilitates next steps, as appropriate.

Scope: The scope of this policy applies to all university administrators, faculty, staff, and students.
