Policy: The University may use the name, likeness, image, voice, and/or appearance as recorded in any media format, including but not limited to photographs, video recordings, and audio recordings, for educational, promotional, marketing, and informational purposes that the University deems appropriate.

The University may use any student content, materials, or curriculum items created, contributed to, or participated in as part of their affiliation with the University to be used, adapted, modified, published, and distributed by the University for educational, promotional, and advertising purposes.

The University or its authorized affiliates, such as faculty, subject matter experts, consultants, and contractors, may use the materials mentioned within this policy in print, digital, website entries, social media, and broadcast channels.

The University will seek student authorization to use the materials mentioned above by completing at least the Media Release and Consent Authorization Form.

Purpose: This policy is intended to obtain a student's written consent to use their work and/or other listed references within the policy.

Procedure:

Any University employee seeking a student’s authorization to use their work, material, or any other item listed within this policy should have the Media Release and Consent Authorization Form completed.

The form must be completed and signed using a black or blue ink pen or an approved electronic signature platform (e.g., AdobeSign, DocuSign). The final signed form must be emailed to the Office of the Registrar for record-keeping at registrar@usuniversity.edu.

Scope: This policy applies to all USU employees.
